By validating the consent form below, I acknowledge having understood and freely given my informed consent to take part in the Study.

I have read and understood the information provided and I accept the terms of the Study's data collection. I have understood in particular that it has an exclusively scientific purpose and in no way constitutes an incentive to use psychedelic substances. I have understood that I can withdraw my contribution from the Study without justification, by my own means using the RIN (Random Identification Number) given at the last page of the questionnaire, for a period of 2 years. After that, I will need to contact the scientific leads, which will lift my anonymity with them. I have understood that I may contact one of the Study's scientific leads with any question, or to withdraw my data. I have understood that by taking part in the Study I agree for an anonymous copy of my contribution to be collected by the scientific leads and used for research purposes. I accept that my anonymous data is stored on a secure UniCA server and managed by the Study's scientific leads. I am aware of the limits of anonymity if elements of my narrative have already been made public elsewhere (forums, social media). I acknowledge being of sound mind, not currently under the influence of a substance, and not under guardianship or influence of anyone to take part in the Study.
